The 8 Facial Treatments in Member’s Wallet and Membership inclusions will expire in 12 months. We
suggest that patient’s redeem the Facials every 6 weeks.

•  The monthly membership fee is $99 per month along with a one-time, non-refundable, $99 enrollment cost. The enrollment cost is paid yearly . Monthly membership fees are drafted on a 30-day rolling basis and are typically withdrawn on the same calendar day each month.

•  If the 8 facial treatments are not used within 12 months they will automatically expire.

•  Rejuv Facial Medical Membership is a 12 month term with NO automatic renewal. To re-enroll after 12 months please sign-up online www.rejuvma.com/membership or contact rejuv@rejuvma.com Memo: Enroll in Facial Membership.

• To cancel The Facial Membership before the 12 months the Member must have moved more than 50 miles from any Rejuv Medical location and will be required to send a current utility bill (gas, water, or electricity) with the address and Member name on listed on the bill. If a Member has not relocated, and hasn’t been a Member for the 12 month minimum, a $149 short term cancellation fee will be due to terminate Membership. All cancellation requests must be sent to rejuv@rejuvma.com Memo: Membership Cancellation (with the supporting documentation if applicable).

•  The monthly Membership starts the day you sign up for your Membership. For example, if you sign up on January 16th, 2022, you have until January 15th, 2023 to use your Member benefits.

• Membership pricing, promotions and offers are non-transferrable and are for the Member’s use only, unless otherwise noted.

•  Membership discounts do not apply to training, modeling, and flash sale specials.

•  Monthly Membership dues cannot be supplemented by any other promotional vendor programs such as Alle or Aspire Rewards, but if a Member for example purchases SkinMedica product or treatment, they get the Member rate, plus they can use their Alle points.

• Any Membership fee that is returned for insufficient funds, incorrect card number or card information is considered in arrears and due to Rejuv Medical Aesthetics. If a Membership is considered in arrears, Membership services and discounts will not be available for use until balance is paid up to and including the current month due.

•  Membership fee cannot be offset by any in store credit and must be paid by a Visa/Mastercard/American Express/Discover.

•  Monthly Membership fees are drafted on a 30 day rolling basis and are typically withdrawn on the same calendar day each month.

• Particular appointment availability or provider availability is not and cannot be guaranteed. All of our providers go through our rigorous training process.

•  Rejuv Medical Members agree to abide by Rejuv. Medical Policies.

•  Membership payments made are not refundable.
