REJUV GOLD MEMBERSHIP TERMS AND CONDITIONS
MONTHLY MEMBERSHIP INCLUDES:

• 10% off services, treatments, products, and skincare.

• Extra $50 off Dermal Filler/Kybella/Sculptra

• $12 Botox, Dysport, or Xeomin

•  Member only promotions and/or events

Monthly Membership Fee is placed in Patient's Rejuv. online 'wallet'. Membership Fee accrued in wallet
expires after 6 months.  Reminder's are sent to patient's email/text 14 days prior that the Member’s
Wallet will expire.

•  The monthly membership fee is $149 per month along with a one-time, non-refundable, $19 enrollment fee. The enrollment fee is paid only once as long as the Membership remains current. Monthly membership fees are drafted on a 30-day rolling basis and are typically withdrawn on the same calendar day each month.

• The maximum membership amount that may be accumulated is 6 months’ worth of fee placed in Membership Wallet or $894 ($149 x 6). The banked funds can be used towards any Rejuv Medical service or product. If the funds are not used and your account reaches $894, the funds automatically expire.

• Rejuv Gold Medical Membership is a 6 month term, which renews automatically unless the notice below is provided.

• To cancel a Membership after the 6 month minimum a 30 day written notice is required. In order to cancel a Membership before the 6 month minimum term has lapsed, the Member must have moved more than 50 miles from any Rejuv Medical location and will be required to send a current utility bill (gas, water, or electricity) with the address and Member name on listed on the bill. If a Member has not relocated, and hasn’t been a Member for the 6 month minimum, a $149 short term cancellation fee will be due to terminate Membership. All cancellation requests must be sent to rejuv@rejuvma.com Memo: Membership Cancellation with the supporting documentation.

• The monthly Membership starts the day you sign up for your Membership, and renews on that date each month. Your Member discounts and benefits renew on that date as well. For example, if you sign up on January 16th, 2022, you have until June 30th, 2022 to use your monthly Member payment benefit.

• Membership pricing, promotions and offers are non-transferrable and are for the Member’s use only, unless otherwise noted.

• Membership discounts do not apply to training, modeling, and flash sale specials.

• Monthly Membership dues cannot be supplemented by any other promotional vendor programs such as Brilliant Distinctions or Aspire Rewards, but if a Member for example purchases Dysport, they get the Member rate, plus they can use their Aspire points.

• Any Membership fee that is returned for insufficient funds, incorrect card number or card information is considered in arrears and due to Rejuv Medical Aesthetics. If a Membership is considered in arrears, Membership services and discounts will not be available for use until balance is paid up to and including the current month due.

• Membership fee cannot be offset by any in store credit and must be paid by a Visa/Mastercard/American Express/Discover.

• Membership funds, the balance of a member’s virtual wallet, cannot be transferred to the virtual wallet of another member or regular Rejuv Medical Aesthetics client.

• Monthly Membership fees are drafted on a 30 day rolling basis and are typically withdrawn on the same calendar day each month.

• Particular appointment availability or provider availability is not and can not be guaranteed. All of our providers go through our rigorous training process.

• Rejuv Medical Members agree to abide by Rejuv Medical Policies.

• Membership payments made are not refundable.
